Update Your Signature

When you return your ballot by mail, your signature is used to verify your identity. In order for your vote by mail ballot to count, the signature on your ballot must match the signature on your voter file or your ballot will not count.

It's a good idea to send a signature update to the Supervisor of Elections.

  1. Print this form (click here)
  2. Check the "Record Update" box
  3. Complete the name and date of birth boxes and sign in the signature box at the bottom 
  4. Mail the form to the Supervisor of Elections at 4455 Avenue A, Suite 101 - St. Augustine, FL 32095

If you're unable to download the form, just mail us a letter with your name, date of birth, and signature. Make sure you mention that this is a record update in your letter.
